Sagging skin is one of the first symptoms of ageing of the face, often from the age of 40. The process can be explained in two ways:

• Aging of the skin, marked by loss of elasticity (less elastin fiber) and firmness (less collagen fibers). By losing its mechanical properties, the skin is vulnerable to gravity, which causes the structures to sag down.
• Changes in volume, as excess fat will stretch the skin. When this fat melts (diets, weight changes, age...), the skin stays loose as it no longer has enough elasticity.

A less toned skin will experience a sagging of the volumes.
This is apparent for the oval of the face in the shape of an inverted V. Dermal or fat melting enhances this structural change, because the skin envelope is no longer tight.

For the face, the action of the threads lift includes two complementary mechanisms, biological and mechanical. The physician inserts a thread under the surface of the facial skin to redefine the anatomical contours to smooth out the wrinkles. There is an instant mechanical act of tension by traction. The physician doses it before stitching. The threads underneath the skin will be absorbed, but they stimulate the release of growth factors and the activation of stem cells. New triggered fibroblasts can induce neocollagenesis with a regenerative effect: this is the biological inducing action.

A good thread lift must implant the thread in the right tissue (often superficial hypodermis) so that the skin is stretched without being visible.
